INSURANCE INFORMATION
We are currently in network providers for Medicare, Blue Shield and Cigna. If you do not see your insurance provider listed as in network we can provide you with a super bill that you can submit to your insurance for reimbursement.
Cash Rates
If your insurance is out of network, our cash rate is $250 for an evaluation and $150 for a treatment. We will provide you with the super bill to submit to your insurance for reimbursement.
