INSURANCE INFORMATION
We are currently in network providers for Medicare and Blue Shield. We believe you deserve the best possible Physical Therapy to achieve your recovery goals. To maintain our high standards of care and provide the dedicated attention you need, Rise Physical Therapy and Pilates operates with a limited number of in-network insurance plans and offers cash-pay options. This allows us to prioritize your treatment above all else. If you have out-of-network benefits, we will provide you with a detailed superbill for submission to your insurance company.
Cash Rates
Our standard cash rates for out-of-network clients are $250 for the initial evaluation and treatment, and $150 for follow-up sessions with most of our Physical Therapists, including most of our vestibular PT's. For vestibular care with Dr. Kim Colón, our advanced vestibular specialist, the cash rates are $300 for the initial evaluation & treatment and $200 for subsequent treatments.
